La méthode Kangourou a été initiée par des pédiatres colombiens en 1979. Elle consiste en un portage des nouveau-nés de petit poids de naissance, peau à peau contre la poitrine de leur mère, 24 heures sur 24. Elle est appliquée aujourd'hui dans de nombreaux pays en développement où elle permet de compenser le manque de couveuses tout en offrant des avantages par rapport aux soins classiques : équilibre thermique des nouveau-nés, durée prolongée de l'allaitement maternel, bénéfices psychologiques en favorisant le lien mère-enfant, amélioration de la survie. La méthode kangourou a été adaptée en Europe sous la forme de soins kangourou avec portage peau à peau des nouveau-nés de petit poids de naissance par leur mère quelques heures par jour. On voit également apparaître des unités kangourou consistant à mettre la couveuse dans la chambre de la mère, ce qui lui permet d'avoir un contact précoce avec son enfant et de participer aux soins. Ces méthodes ont le grand intérêt de privilégier l'interaction parents-enfant. Elles nécessitent toutefois un personnel qualifié, très disponible, formé et disposé à assurer cette nouvelle approche et en prendre en compte sa dimension psychologique.
Summary
The kangaroo-mother method was initiated by colombian pediatricians in 1979. This method is based on a permanent skin to skin contact of low birth weight infants with their mother. It has spread out in many developping countries as an alternative cheap method for the care of low birth weight infants with several advantages: temperature regulation, prolonged breast-feeding, promotion of mother-infant interaction, decreased mortality. The kangaroo method has been adapted in European countries as kangaroo care that consists in daily mother-infant skin to skin contact during few hours. Introducing the incubator in the mother's room is an other derivative of the method which allows prolonged early contact of the mother with her infant. A major interest of these methods is that they favour parent-infant interaction; however this requires qualified and devoted staffs.

Early developmental interventions with premature newborns: Room for improvement
2020, KinesitherapieLa prématurité nécessite chez de nombreux bébés des interventions précoces de développement adaptées. Les masseurs-kinésithérapeutes ont aujourd’hui un arsenal thérapeutique diversifié pour influer sur leurs évolutions motrices. Cependant, la rapidité de prise en charge est un facteur clé, et l’organisation des soins intra-hospitaliers aussi bien que la coordination avec les soins de ville peuvent être une limite.
Déterminer les facteurs ralentissant cette prise en charge rééducative.
Des entretiens ethnographiques ont été réalisés auprès de mères de bébés prématurés ayant une indication de rééducation, ainsi que des observations ethnographiques réalisées en néonatalogie et dans les consultations médicales hospitalières de suivi.
L’organisation hospitalière peut entraîner des priorisations des soins vitaux immédiats au détriment des soins de l’ordre du care, à la fois moins valorisés par l’organisation hospitalière mais aussi exercés par des personnels dominés dans ce milieu.
Une horizontalisation des relations interprofessionnelles pourrait sans doute amener une diversification des priorités hospitalières et une meilleure prise en charge des nouveau-nés prématurés.

Prematurity requires appropriate early developmental interventions for many babies. Physiotherapists today have a diversified therapeutic arsenal to influence motor development. However, early care is a key factor, and the organization of hospital care and coordination with community care can be limitations.
To determine the factors hindering rehabilitative care.
Ethnographic interviews were carried out with mothers of premature babies with indication for rehabilitation. Also, ethnographic observations were made in the neonatal department and in hospital medical follow-up consultations.
Hospital organization may prioritize immediate life-saving treatment to the detriment of care as such, which is both less valued by the hospital organization and also is provided by staff who are low in the hierarchy of this environment.
Horizontalization of interprofessional relations could undoubtedly diversify hospital priorities and improve management of premature newborns.

Impact of the Kangaroo “Supported diagonal flexion” positioning on mother's spontaneous touch during the very first skin-to-skin session in NICU
2017, Neuropsychiatrie de l'Enfance et de l'AdolescenceLa présente étude s’intéresse au maternage tactile spontané lors du premier peau à peau réalisé avec de grands prématurés en service de réanimation néonatale. L’objectif est de tester l’hypothèse selon laquelle, comparée à l’installation usuelle où le bébé est installé verticalement dans les vêtements de la mère entre ses 2 seins, l’installation novatrice en « Flexion diagonale soutenue » (FDS) est davantage favorable à un maternage tactile de qualité, à visée interactive, et moins centré sur le maintien postural du bébé.
L’échantillon comprend 20 grands prématurés nés entre 27 et 31 semaines et 6 jours et leurs mères. Les 10 premiers ont été installés en peau à peau selon la méthode couramment pratiquée in situ, en verticale avec coussin d’allaitement, les 10 suivants ont été installé en Flexion diagonale soutenue. Les 5 premières minutes du tout premier peau à peau ont été filmées et codées avec le logiciel The Observer. Le risque de dépression maternelle a été évalué au moyen de l’EPDS et les états de conscience du bébé au moyen de l’échelle APIB.
Dans l’installation usuelle en verticale, les mères passent 59 % du temps à maintenir la posture de leur nouveau-né contre seulement 1 % dans l’installation FDS (IC 95 % 47,2–67,2). Dans l’installation FDS, libérées d’un toucher fonctionnel, les mains de la mère se dédient de façon équilibrée au toucher présence (39 %, contre 21 % en installation verticale, IC 95 % 5,3–30,0) et au toucher affectif (36 %, contre 16 % en installation verticale ; IC 95 % 7,8–31,4). Enfin, en FDS, les prématurés sont principalement en sommeil léger avec une somnolence diminuée et une absence d’éveil actif et agité.
Spontanément, en installation FDS, les mères manifestent un maternage tactile plus actif, davantage affectif, favorisant une rencontre précoce de qualité dès le tout premier peau à peau.
The present study focuses on mother's spontaneous touch during the very first skin to skin kangaroo care with premature babies in NICU. The aim is to test the hypothesis according to which, compared to the usual installation where the baby is installed vertically in the mother's clothes between the breast, the innovative installation in Supported diagonal flexion (FDS) fosters a more positive qualitative mother's touch, a touch dedicated more to enriching and scaffolding interactions with the preemie and less focused on postural support.
Twenty very preterm infants between 27 and 31 weeks gestation and their mothers were offered skin to skin sessions. The first 10 infants included were offered the usual vertical positioning and a breastfeeding pillow for support. The following ten were offered SDF. The first five minutes of the session were videotaped and coded with The Observer software.
In the usual vertical positioning mother's hands were used 59% of the time to maintain infant posture (hold or contain infant's body), therefore using touch for mechanical reasons (vs 1% in SDF positioning; 95% CI 47.2–67.2). With the SDF positioning, mothers used their hands for affective touch (36% vs 20% in the usual vertical positioning; 95% CI 5.3–30.0) and to signal presence (39% vs 20% in the usual vertical positioning; 95% CI 7.8–31.4). Infants were mostly in active sleep, and drowsiness was lowered in the FDS group as compared to the “as usual” one. There was no quiet awake or active awake state during skin to skin's SDF positioning sessions. Our results also showed that affective touch took place on the head in the vertical group whereas it was more frequently directed on the face in the SDF group. Mothers in the SDF group tended spontaneously more to rock their baby.
With SDF positioning, mothers spontaneously used more active touch and expressed more affection through touch. The present study is part of a longitudinal follow-up from birth until 3 months corrected age. The vocal production and visual engagement of both partners are under analysis and will be considered in the light of the mothering touch interactions described here. Further research to describe interactions in all of their dimensions is necessary.
Kangaroo supported diagonal flexion positioning: New insights into skin-to-skin contact for communication between mothers and very preterm infants

[7] pointed out the influence of skin-to-skin contact on the maturation of the autonomic nervous system and the establishment of circadian rhythms in premature infants. Early skin-to-skin contact may foster vulnerable newborn children's adaptation to extrauterine life, enhance short- and long-term maternal (parental) care, and initiate early positive interactions [7,8]. The positive effects of skin-to-skin contact–not only on the infant's development but also on the parents’ practices and perceptions during the first 6 months of life–are now well-known [7].
Skin-to-skin contact shows benefits in the relationship developed between a mother and her premature infant. In the skin-to-skin session, face-to-face exchanges are impossible in vertical infant positioning. We therefore undertook an observational, prospective, single-center study using kangaroo “supported diagonal flexion” (SDF) positioning. The first aim was to evaluate the safety of kangaroo SDF positioning compared to the usual vertical positioning. The second aim was to evaluate SDF positioning on early communication between the mother and her infant and to improve their well-being.
Fifteen mothers and their very premature infants (birth 26 < 32 weeks’ gestation) were assigned to one of the two kangaroo positioning modes, either the current vertical positioning (n = 7) or SDF positioning (n = 8).
Physiological variables and critical events were recorded before, during, and after ten successive skin-to-skin contact sessions. The first and last sessions were videotaped to allow later behavioral measurements. Mothers’ risk for depression and feelings about the way they experienced communication with their infant were assessed through questionnaires.
In terms of the infant's physiology, no negative effects were associated with SDF positioning in comparison with the usual vertical positioning. SDF positioning led to fewer disorganized gestures, negative vocalizations, and drowsiness, in favor of more deep sleep. SDF led to more mother–infant eye-to-eye contact as well as maternal vocalizations, smiles, and caressing, although these differences did not reach significance. The score for the risk of postnatal depression decreased significantly between the first and the last session in the SDF group, whereas it did not change in the vertical positioning group.
These results support the idea that the kangaroo SDF positioning technique is physiologically safe, has obvious immediate benefits on mothers’ infant-directed communicative behaviors, and respects the baby's naturally flexed and asymmetrical tonic neck posture. It is an innovative, inexpensive, easy-to-use technique in daily practice, by all healthcare professionals working in a neonatal intensive care unit. These data suggest that the current kangaroo positioning technique could be improved. More studies are needed to confirm the benefits and safety of the kangaroo SDF positioning in larger groups of preterm infants.
Le peau-à-peau est bénéfique à la relation entre la mère et le nouveau-né prématuré. Il est réalisé habituellement en position verticale qui présente l’inconvénient de rendre difficile l’interaction visuelle entre la mère et l’enfant. Nous avons mené une étude observationnelle prospective et monocentrique. Notre premier objectif était de vérifier qu’une nouvelle installation, la flexion diagonale soutenue (FDS) était physiologiquement sûre par rapport à l’installation habituelle. Le second objectif était d’évaluer l’hypothèse d’une meilleure interaction visuelle, au sein des interactions précoces, et un bien-être respectif de la mère et du prématuré, avec l’installation FDS.
Quinze mères et leurs bébés nés très prématurément (entre 25 et 32 semaines d’aménorrhée), hospitalisés dans un même centre, ont bénéficié du mode d’installation vertical (n = 7) ou du mode FDS (n = 8). Les paramètres physiologiques et événements critiques ont été enregistrés avant, pendant et après chacune des 10 séances successives de peau-à-peau. Les interactions ont été filmées à la première et dernière séance. Le bien-être a été évalué chez la mère par un auto-questionnaire de risque de dépression et chez le prématuré par son comportement.
Chez le prématuré, aucune différence physiologique n’a été observée entre les deux types d’installation. L’installation FDS a conduit à moins d’agitation motrice, moins de vocalisations négatives, moins de somnolence, plus de sommeil profond que l’installation verticale. Chez la mère, l’installation FDS a conduit à plus d’attention visuelle, de vocalisations et de sourires dirigés vers son enfant et un risque de dépression maternelle diminué, par rapport à l’installation verticale.
Ces résultats montrent que l’installation FDS semble physiologiquement sûre. Elle a des effets positifs immédiats sur les premières interactions mères–prématurés et respecte la posture naturellement fléchie et asymétrique adoptée par le nouveau-né. C’est une technique psychomotrice innovante, non coûteuse et facilement reproductible dans la pratique quotidienne des professionnels en médecine néonatale. Ces données suggèrent que la technique de positionnement en peau-à-peau pourrait être améliorée. D’autres études doivent confirmer la sécurité et les bénéfices de l’utilisation de l’installation FDS auprès d’un échantillon plus important de grands prématurés.
Prematurity and parental post-traumatic stress disorder: Similarities and differences

En fonction du score de risque périnatal (PERI) du bébé, les parents des prématurés (âge gestationnel moins de 34 semaines) ont été divisés en deux groupes : les parents de prématurés à faible risque (n = 16) et à haut risque (n = 26). Les symptômes d’intrusion et d’évitement, de l’état de stress post-traumatique, ont été évalués chez les parents à l’aide d’un questionnaire, l’Impact of Event Scale (IES). Leurs réponses ont été comparées à un groupe témoin de parents de nouveau-nés à terme (n = 24). Les différences entre les réponses des mères et des pères, ont été analysées.
Les parents de bébés prématurés sont plus à risque que les parents de nouveau-nés à terme de présenter des symptômes de stress post-traumatique. Les mères en lien avec le fait même de la prématurité du bébé, les pères en lien avec la gravité de la prématurité. Les mères et les pères des prématurés des deux groupes (prématurés à faible risque, prématurés à haut risque) décrivent des symptômes d’intrusion, alors que les symptômes d’évitement sont décrits par toutes les mères, mais seulement par les pères de prématurés à haut risque périnatal.
La naissance prématurée est susceptible d’entraîner l’apparition de symptômes de stress post-traumatique chez les parents. Les mères et les pères réagissent différemment.
Evaluation of the symptoms of parental post-traumatic stress disorder (PTSD), according to the severity of the prematurity, in mothers and fathers of premature babies.
According to the Perinatal Risk Inventory (PERI), the parents of premature infants (gestational age less than 34 weeks) were divided into two groups, parents of a low-risk premature infants (n = 16) and of high-risk premature infants (n = 26). The symptoms of intrusion and avoidance, as a part of the post-traumatic stress disorder, were evaluated by an autoadministrated questionnaire, the Impact of Event Scale (IES). Their responses were compared with a control group of parents of full-term infants (n = 24). The differences in the answers of mothers and fathers were analysed.
The occurrence of symptoms of post-traumatic stress disorder is increased in parents of preterm infants compared with the control group. Whereas mothers of premature infants are at risk of presenting symptoms of PTSD, linked to the prematurity, with fathers the infant perinatal risk factors play a greater role. The symptoms of intrusion are present in mothers and fathers of preterm infants of both groups. Mothers of both groups present avoidance symptoms, although only fathers of high-risk preterm infants present them.
Premature birth has an impact on both parents in terms of post-traumatic stress reactions. However, mothers and fathers react in different ways according to the severity of the prematurity.
Kangaroo mother care: Bibliographical review on the current attitudes, their interests and their limits
2004, Archives de PediatrieInitiés en 1978 par une équipe colombienne, puis largement repris aussi bien dans les pays industrialisés que dans les pays en voie de développement, les unités et soins kangourou sont connus pour assurer aux nouveau-nés hypotrophes quel que soit leur terme, une thermorégulation, une stabilité physiologique et un meilleur confort relationnel avec leurs parents. Le but de ce travail est de faire une revue bibliographique sur les concepts actuels, les intérêts et les limites de cette méthode. Nous avons successivement revu l'impact de la méthode kangourou sur le métabolisme basal, la thermorégulation, la croissance pondérale et le devenir des enfants. L'intérêt paraît indiscutable dans les pays en voie de développement, mais ne devrait en aucun cas faire oublier la sécurité médicale. Dans ce contexte de ressources matérielles limitées avec une fréquentation élevée des services, ils sont capables d'assurer aux nouveau-nés de petit poids de naissance une normothermie et une adaptation métabolique basale. Dans les pays industrialisés, les soins kangourou contribuent à diminuer l'angoisse des parents et à améliorer les relations privilégiées avec leur enfant. Toutefois, il est difficile de recommander leur usage en pratique courante car des essais randomisés rigoureux sont nécessaires pour argumenter leur implantation en toute sécurité, connaître le développement neuropsychologique et la croissance somatique réelle sur le long terme de ces enfants, maîtriser leur véritable coût économique.
Initiated in 1978 by a Colombian team, then largely adapted in industrialized countries as well as in poor developed countries, the kangaroo mother care (KMC) are known to ensure for low birth weight newborn, a thermoregulation, a good physiological stability and a better relational comfort with their parents. The goal of this work is to make a bibliographical review on current concepts, interests and limits of this method. We re-examined impact of the KMC on the basal metabolism, thermoregulation, growth and evolution of these children. They are helpful in the developing countries but medical safety should not be forgotten. In these countries where there’s high frequentation of the services, they are able to regulate body temperature and metabolic adaptation of the newborn. In developed countries, KMC contribute to decrease anxiety of parents and improve the relations with their child. However, it is difficult to recommend their use in current practice. Rigorous randomised studies are necessary to argue their establishment in full safety, to know the neuropsychological development and the real somatic growth on the long term of the children and to known their true economic cost.
An innovative position during skin-to-skin contact in neonatology

Skin-to-skin is, by its very nature, the only relational care entirely devoted to the parent-baby relationship, from the infant's time in neonatal intensive care. However, current practice is unsuited to visual and tactile exchange. A simple change to the baby's positioning helps to improve the quality of the immediate interaction between the infant and the mother, benefiting the development of the baby and parenthood.
