2020, Seminars in Fetal and Neonatal MedicineCitation Excerpt :In 1982, Kovar et al. first introduced the use of serum ALP as a biomarker to test for rickets in preterm infants and suggested a cutoff of five times the upper limit of the normal range in adults (>1650 U/L). This was based on the serum ALP levels of four of 30 infants who had radiologic evidence of rickets [11,12]. In 1982, Koo et al. also reported correlations between decreased skeletal demineralization on X-rays and elevated ALP level, decreased birth weight, decreased gestational age, and decreased enteral feeds.

The levels of ALP and P are the most commonly used biochemical markers for the detection of MBD [6,8,12,13]; however, there is no clear cut-off value reported for an accurate diagnosis of MBD [14,15]. Rustico et al. [16] showed that only the serum ALP level was increased in a fracture group compared to an osteopenia group, and there were no significant differences in serum P, calcium, or parathyroid hormone (PTH) levels.

TALP when elevated is a marker of metabolic bone disease and is elevated in children with rickets [23]. The increased TALP concentrations in the LI group may be indicating that poor iron status has a negative influence on bone health, possibly mediated through its association with elevated FGF23 production.

[15] suggest that an ALP activity that is five times higher than the upper limit of the normal adult reference range is acceptable in preterm infants, but an activity higher than this may suggest rickets. In addition, the male sex is also a risk factor of rickets.
