Two papers in particular have shown this not to be the case and describe it as ineffective at reducing the bulk of haemangiomas, although it has a role in the later cosmetic management of residual telangiectasia.12,13 Despite an excellent randomised controlled study showing its ineffectiveness,13 other authors continue to use laser therapy as primary management in select cases.54,55,75,76 Very early treatment of purely superficial haemangiomas with flashlamp-pumped pulsed dye laser (FPDL) therapy is advocated in some centres to prevent the development of a subcutaneous component, with the authors describing excellent results in carefully selected cases, but resorting to surgical debulking for resistant lesions in patients with a deeper component, diagnosed by ultrasound.75
