Cordocentesis has shown that antenatal hypoxia87-89 (eg, because of growth retardation and multiple pregnancy) is associated with an increased incidence of stillbirth, metabolic acidosis during labor, and subsequent abnormal neurodevelopment.89 Although this clinical experience strongly suggests that such infants are likely to be compromised by otherwise well-tolerated labor, intriguingly, experimental studies seem to suggest improved or greater cardiovascular adaptation to moderate induced hypoxemia.

The hypoxic group were smaller at postmortem evaluation and had a mild but significant basal acidosis with hypercarbia and elevated lactate and lower blood glucose levels, which is consistent with significant chronic placental impairment and similar to clinical observations.1-3 It is possible that the differences from previous studies might relate to other secondary features that are associated with hypoxia that might not be seen in models of induced hypoxia in normally grown fetuses (such as chronic partial restriction of the umbilical cord).
