AB  - Patient safety incidents (PSIs) occur relatively frequently in healthcare. Incident-reporting systems are designed to systematically collect information relating to PSIs in order to identify and rectify problems in the delivery of clinical care. This review provides an overview of the incident reporting process and summarises local and national data of PSIs reported in a neonatal population.
