Abstract

Patient safety incidents occur relatively frequently in healthcare. Incident reporting systems are designed to systematically collect information relating to patient safety incidents in order to identify and rectify problems in the delivery of clinical care. This review provides an overview of the incident reporting process and summarises local and national data of patient safety incidents reported in a neonatal population.
