A term male infant weighing 2.75 kg was referred to the neonatal team due to concerns about small penile size (figure 1). On review he had a stretched penile length of 2 cm, which is a borderline measurement for micropenis but not conclusive (box 1).1–4 He had descended testes, no dysmorphic features or midline defects. During the examination he was also noted to be lethargic; blood glucose was measured and was found to be 0.6 mmol/L.
