The MRC trial led to the recommendation that all women planning a pregnancy should consume 0·4 mg folic acid per day (the dose used in the Smithells trials), and that women at high risk of having a baby with an NTD should receive 4–5 mg per day. However, despite widespread public health education efforts in the UK and other countries, the incidence of NTDs did not decrease during the decade after publication of the MRC trial.25 In the USA, a campaign of government lobbying26 eventually achieved its aims, and mandatory fortification of bread flour with folic acid was introduced in 1998.
