Abstract

The effect of ethamsylate, a capillary-stabilising drug, in limiting or preventing periventricular haemorrhage (PVH) in 70 very low birth-weight babies was evaluated in a double-blind trial. PVH developed in 9 of the 35 infants on ethamsylate and 18 of the 35 infants who received placebo. Mortality associated with PVH was similar in both groups, but the incidence of PVH of all grades in survivors was reduced in the ethamsylate-treated group.

These cumulative observations indicate that extremes in PaCO2 should be avoided during the period in which infants are at high risk of IVH. In the early 1980’s postnatal medications used to prevent hemorrhage included phenobarbital,40,41, Vitamin E,42,43 and ethamyslate.44 While there was initial enthusiasm for the use of these medications, this was not borne out over time.

These infants present the closest clinical parallel to term infants who have acute encephalopathy, and thus are the most likely to benefit while being at the least risk for hypothermia-associated complications. Postnatal administration of medications to reduce severe IVH has historically included the use of phenobarbital,55–57 vitamin E,58 ethamyslate,59 and indomethacin.60 This IVH prevention armamentarium has not borne out over time, with the exception of indomethacin, which is discussed first.

By contrast, the risk for severe PV–IVH in the nonintubated infant is low, <10%.89 Postnatal medications used to prevent hemorrhage have included phenobarbital,90-92 vitamin E,93 ethamyslate,94 and indomethacin.95-97 Initial enthusiasm for the use of the first 3 medications has not been borne out over time.
