A simplified scoring system for clinically determining fetal maturation of newly born infants has been developed and provides accurate assessment of gestational age in either well or sick babies. Certain conditions render individual criteria within the score less reliable but do not significantly lessen the reliability of the total assessment. The optimal age for maturational assessment is between 30 and 42 hours of age.

Associations of early childhood body mass index trajectories with body composition and cardiometabolic markers at age 10 years: the Ethiopian infant anthropometry and body composition (iABC) birth cohort study

Variability in body mass index (BMI) (kg/m²) trajectories is associated with body composition and cardiometabolic markers in early childhood, but it is unknown how these associations track to later childhood.
We aimed to assess associations of BMI trajectories from 0 to 5 y with body composition and cardiometabolic markers at 10 y.
In the Ethiopian infant anthropometry and body composition (iABC) birth cohort, we previously identified 4 distinct BMI trajectories from 0 to 5 y: stable low BMI (19.2%), normal BMI (48.8%), rapid growth to high BMI (17.9%), and slow growth to high BMI (14.1%). At 10 y, we obtained data from 320 children on anthropometry, body composition, abdominal subcutaneous and visceral fat, and cardiometabolic markers. Associations of BMI trajectories and 10-y outcomes were analyzed using multiple linear regression.
Compared with children with the normal BMI trajectory, those with rapid growth to high BMI had 1.7 cm (95% CI: 0.1, 3.3) larger waist circumference and those with slow growth to high had 0.63 kg/m² (95% CI: 0.09, 1.17) greater fat mass index and 0.19 cm (95% CI: 0.02, 0.37) greater abdominal subcutaneous fat, whereas those with stable low BMI had −0.28 kg/m² (95% CI: −0.59, 0.03) lower fat-free mass at 10 y. Although the confidence bands were wide and included the null value, children with rapid growth to high BMI trajectory had 48.6% (95% CI: −1.4, 123.8) higher C-peptide concentration and those with slow growth to high BMI had 29.8% (95% CI: −0.8, 69.8) higher insulin and 30.3% (95% CI: −1.1, 71.6) higher homeostasis model assessment of insulin resistance, whereas those with rapid growth to high BMI had −0.23 mmol/L (95% CI: −0.47, 0.02) lower total cholesterol concentration. The trajectories were not associated with abdominal visceral fat, blood pressure, glucose, and other lipids at 10 y.
Children with rapid and slow growth to high BMI trajectories before 5 y tend to show higher measures of adiposity and higher concentrations of markers related to glucose metabolism at 10 y.

Associations of weight and body composition at birth with body composition and cardiometabolic markers in children aged 10 y: the Ethiopian infant anthropometry and body composition birth cohort study

Although birth weight (BW) has been associated with later cardiovascular disease and type 2 diabetes, the role of birth fat mass (BFM) and birth fat-free mass (BFFM) on cardiometabolic health is unclear.
To examine associations of BW, BFM, and BFFM with later anthropometry, body composition, abdominal fat, and cardiometabolic markers.
Birth cohort data on standardized exposure variables (BW, BFM, and BFFM) and follow-up information at age 10 y on anthropometry, body composition, abdominal fat, and cardiometabolic markers were included. A linear regression analysis was used to assess associations of exposures with outcome variables, adjusting for maternal and child characteristics at birth and current body size in separate models.
Among 353 children, mean (SD) age was 9.8 (1.0) y, and 51.5% were boys. In the fully adjusted model, 1-SD higher BW and BFFM were associated with 0.81 cm (95% CI: 0.21, 1.41 cm) and 1.25 cm (95% CI: 0.64, 1.85 cm) greater height at 10 y, respectively. The 1-SD higher BW and BFM were associated with 0.32 kg/m² (95% CI: 0.14, 0.51 kg/m²) and 0.42 kg/m² (95% CI: 0.25, 0.59 kg/m²) greater fat mass index at 10 y, respectively. In addition, 1-SD higher BW and BFFM were associated with 0.22 kg/m² (95% CI: 0.09, 0.34 kg/m²) greater FFM index, whereas a 1-SD greater BFM was associated with a 0.05 cm greater subcutaneous adipose tissue (95% CI: 0.01, 0.11 cm). Furthermore, 1-SD higher BW and BFFM were associated with 10.3% (95% CI: 1.4%, 20.0%) and 8.3% (95% CI: −0.5%, 17.9%) greater insulin, respectively. Similarly, 1-SD higher BW and BFFM were associated with 10.0% (95% CI: 0.9%, 20.0%) and 8.5% (95% CI: −0.6%, 18.5%) greater homeostasis model assessment of insulin resistance, respectively.
BW and BFFM rather than BFM are predictors of height and FFM index at 10 y. Children with higher BW and BFFM showed higher insulin concentrations and homeostasis model assessment of insulin resistance at 10 y of age.

Care of the Newborn

The care of a normal newborn is a balance between facilitating maternal-infant bonding and successful transition to extra-uterine life while providing interventions to prevent disease and screening to allow for the early identification of rare conditions. Ideally, evidence-based interventions and monitoring of the newborn infant should be provided as unobtrusively as possible. In this chapter, we examine the evidence surrounding the most commonly recommended interventions, screening, monitoring, and assessment in the care of the newborn. Normal and abnormal adaptation to extra-uterine life including vital signs, temperature regulation, weight loss, stool and urine patterns, and glucose homeostasis are described and suggested management of common newborn problems is provided. The evidence for the benefits of interventions such as intramuscular vitamin K, newborn metabolic, hearing, and pulse oximetry screening is weighed against their potential risks. For controversial topics such as male circumcision and maternal HIV-positive breastfeeding, balanced information is provided that can be used by clinicians for discussions with parents. Finally, two of the central concerns during the newborn stay are jaundice and possible sepsis. While clinicians are most concerned with under-diagnosis of either condition, the effects of overtreatment are considerable. The evidence underlying recommended management guidelines for these conditions is presented, and methods for screening for jaundice and sepsis that limit invasive testing and unnecessary treatment while maximizing the chance of identifying newborns likely to benefit from intervention are offered.
Care in the newborn nursery

The care of a newborn infant within the newborn nursery should be considered a form of well child care. Most newborns are healthy, and there is usually little need for medical interventions other than routine screening and prophylaxis. The widespread availability of comprehensive prenatal care means that many newborn health issues can be detected or anticipated prior to delivery. As such, nurseries should place an emphasis on promoting parental bonding and educating the family on the care of their new arrival rather than making the admission feel overly medicalized as long as the newborn is doing well. In this chapter, the expected hospital course for a newborn is discussed in detail, and the evaluation and management of common medical problems encountered in the newborn period are addressed.
Current epidemiology and factors contributing to postnatal growth restriction in very preterm infants in China

Postnatal growth restriction (PGR) is common in very preterm infants (VPIs) and is associated with adverse short and long-term developmental outcomes. Postnatal growth status for VPIs in middle- or low-income countries remains unclear.
To evaluate PGR in VPIs and identify maternal and neonatal factors, clinical practice, and major neonatal morbidities associated with PGR in China.
Prospective cohort study.
We included 6085 infants born at <32 weeks gestation who were admitted at 57 hospitals in the Chinese Neonatal Network in 2019.
Birth and discharge weights were converted to age-specific Z-scores. PGR was defined as a decrease in weight z-score from birth to discharge >2.
The overall incidence of PGR was 19.9 %. The mean (standard deviation [SD]) weight Z-score was 0.12 (0.78) at birth and decreased to −1.36 (0.98) at discharge. About 4.0 % of VPIs were small for gestational age (SGA) at birth and 25.5 % of SGA infants had PGR. The incidence of PGR increased with decreasing gestational age except in the SGA subgroup. Each 1-unit increase in birthweight Z-score was associated with a 1.49-fold increased risk for PGR. Late initiation of enteral feeds and late achievement of full enteral feeds were positively associated with PGR. The common morbidities that influenced PGR were necrotizing enterocolitis ≥ stage II, patent ductus arteriosus requiring medical or surgical treatment, sepsis, bronchopulmonary dysplasia, and respiratory distress syndrome requiring surfactants.
Nearly one fifth of VPIs were PGR, and one fourth of SGA had PGR, which warranted further study to investigate underlying causes by which to improve postnatal growth in very preterm infants in future.
High level of γH2AX phosphorylation in the cord-blood cells of large-for-gestational-age (LGA) newborns

Newborns can experience adverse effects as a consequence of maternal or in utero exposure, altered growth of the fetus, or placental dysfunctions. Accurate characterization of gestational age allows monitoring of fetal growth, identification of deviations from the normal growth trajectory, and classification of babies as adapted, small, or large for gestational age (AGA, SGA, or LGA). The aim of this work was to evaluate nuclear and oxidative damage in umbilical cord-blood cells of newborns (sampled at birth), by applying the γH2AX assay and the fluorescent probe BODIPY^581/591 C¹¹, to detect DNA DSB and cell membrane oxidation, respectively. No statistically significant differences were observed in the proportion of oxidized cord-blood cells among the groups of newborns, although the LGA group showed the highest value. With regard to genome damage, elevated levels of γH2AX foci were detected in the cell nuclei from LGA newborns as compared to AGA or SGA babies, whose values did not differ from each other. Considering that the observed DNA damage, although still repairable, can represent a risk factor for obesity, metabolic diseases, or other pathologies, monitoring genome and cell integrity at birth can provide useful information for prevention of diseases later in life.
