| 0 - none | No suboptimal care |
| I - minor | Suboptimal care, but different management would have made no difference to the outcome |
| II - significant | Suboptimal care, in which different management might have made a difference to the outcome |
| III - major | Suboptimal care, in which different management would reasonably be expected to have made a difference to the outcome. |
