A 24-week gestational age neonate with severe respiratory distress syndrome was managed with high-frequency oscillatory ventilation and inotropes. Umbilical artery catheter and umbilical venous catheter (UVC) were inserted on day 1. UVC aspirated blood freely at the time of insertion and followed a straight course on abdominal X-ray (AXR). UVC was initially at the eighth thoracic vertebral (T8) level and was pulled back to T10.
